ARI cannot guarantee an outcome. But documentation matters. The more specific the information is, the easier it is to understand what happened, when it happened, where it happened, and what needs to be reviewed.

Include the facility name, person involved, date, time, location, staff names if known, and what happened.

Include Proof

Save screenshots, messages, call logs, letters, grievance numbers, medical notes, and any timeline details.

Avoid exaggeration. Stick to what you know, what you saw, what was said, and what can be documented.
